Multiple factors. (1) Design was 'fracture critical' - no redundancy. One thing breaks, the whole thing goes down. 'We wouldn't build it this way today,' an engineer said. (2) Official report in the aftermath concludes the gusset plates were too thin, a design flaw. (3) Piles of materials (sand & gravel from the looks of it) and construction machinery were on the bridge, and this plus the two lanes of traffic were more weight than the ordinary four lanes of traffic.
